As an alternative to a paper based declaration the following form may be completed on line and the information electronically sent to the Clerk when submitted.

Eligibility to conform with HMRC rules for Gift Aided donations

By completing and submitting this form it is necessary for you to accept the following statements by selecting the appropriate button below and scrolling down to the form:

If you pay income tax at the higher rate, you must include all your Gift Aid donations on your Self Assessment tax return to receive additional tax relief due to you or ask HM Revenue and Customs to adjust your tax code.
